I haven't been able to reproduce, but this would most likely be caused by binding to the top level window in Bug 1145784. Can you reproduce this every time?

No; this was just a one-time thing in the browser toolbox. Have never seen it before, actually

This was almost certainly due to Bug 1167379 - I was just able to reproduce the offset bug when zooming in the Browser Toolbox. Marking as a dupe.
