Ordinarily I wouldn't report this, knowing that there is a lot of old, broken,
javascript out there.  However, this was on http://mva.state.md.us/ 's change of
address form, the first screen worked fine, and a cursory examination of the
screen causing trouble did not reveal why the submission was failing.

Reproducible: Always
Steps to Reproduce:
1. Apply for a Maryland MVA PIN
2. Enter your driver's license, age, and pin into their change of addres s form.
3. Hit submit
4. Sends you to another page, where filling out the information, then hitting
submit does nothing under Mozilla (works in IE)

Alternatively, just look at the attachment, and try getting it to submit (that
is the problem page).

Actual Results:  nothing

Expected Results:  should've submitted my change of address request.

Looking at the source, almost certainly a duplicate of bug 115262 (all the 
inputs seem to be outside the "<form> </form>" tags.

Well, I've already changed my address there - they might get upset if I do it again.
I'll ask the S.O. - should take about a week for her to get her pin. :)

Probably the <form></form> that causes the problem...

Source looks like:
 <form ...></form>
   <input ..>
 </form>

Removing the first </form> fixes it.
