Please re-run diagnostics, since it is failing in the exact same way as before, despite reimaging. (No other machines are hitting this failure).

If diags pass again, I suggest asking iX to do burn-in testing, if this host is under warranty.

memtest ran 39 passes over the weekend, 0 errors. running extended disk diags.

disk diags is taking 6+ hours to complete. opened #EHM-959-73196 with iX for RMA.

picked up RMA from iX and reimaged host. if issues persist we may be looking at a motherboard problem.
