Overview: There are a variety of problems with the Text file Import Dialog

Steps to reproduce:
1. Select File|Import, select the Address Book radio button, select the Text
file option and choose the file
2. When the Import dialog appears notice:

a. It shows the first records but it would be nice to list the total number of
records to get an idea of the ab size.
b. It's overwhelming to see all the checkboxes. It's difficult to quickly
determine what steps should be performed (check or uncheck)
c. It is not obvious that you can drag-n-drop the order of items (i.e.
drag-n-drop 'First Name' after 'Last Name'.
c. After unchecking some boxes I thought I would have to go through each record
and again uncheck the items not wanted. I later found out that this isn't the
case but it wasn't obvious while looking at the first record.
d. There should be a 'check all' option
e. Maybe this dialog can include a more specific dialog title besides Import
Address Book. "Import Address Book (txt, csv...)"

Adding jglick to the Cc: list for additional ideas.

Additionally being able to assign multiple things to a single field would be
handy.  By this I mean having the First name, for instance, come from both the
first name and middle name of the imported account.  If there is a way to do
this, making it easier to know so would be good.
