Also occurs in safe mode with no addons. The email is sent with the header: "From: Firstname Lastname"

I do use Virtual Identity but the problem occurs even when this is disabled. Also occurs in safe mode with all addons disabled. Also occurs with new email message. The 'From' line on the message window only displays "Firstname Lastname" and then "Account Name", i.e. no email address. If I change the email address in the server settings, it is still ignored.
More info: I set up a new profile in 38.1 and the problem did not appear in that. I have looked in the "Identities" section and the problem identities are listed only as "Firstname Lastname" as opposed to "Firstname Lastname <firstname.lastname@example.org>" Changes to the problem identities are accepted but not recognised in v38.1, but work correctly in v31.7. Newly added identities behave correctly. Workaround is to add new identities then delete the old ones.
