According to my testing, it's only reproducible in e10s mode with FF42.

STR:
1) Open http://www.sublimevideo.net/demos/player-designs/flat

First test:
2) Hover over the video poster then move the mouse outside the video

Second test:
3) Start the video and check the first seconds

Result:
2) When you move the mouse outside the video, the end of the opacity transition is wrong and changes suddenly.
3) Less 1 sec after starting the video, a gray overlay appears in the lower right corner of the video (see the screenshot)

Are you on Windows?  If so, maybe try flipping the APZ pref and seeing if that changes things, given that:
https://hg.mozilla.org/mozilla-central/rev/af67b5f73381
is in the regression window.  (See patch for the pref name.)
It works fine with APZ disabled and e10s enabled in FF42.

The URL referenced in comment 0 is no longer around, it seems sublimevideo has shut down. Have you seen this on any other sites?

Let's close this, SublimeVideo has been acquired by Dailymotion and its video player is not more supported.
