Details
If you use the middle mouse button to open an extension install in a new tab, the Allow button is missing.
To recreate:
Go to:
https://www.mailvelope.com/
Click on the download.mailvelope.com link with the middle mouse button.
Switch to the new tab.
Notice that you have no option to enable the instal

Can we at least have a better error message?
As it is, it just looks like the doorhanger is broke or there is something evil about the domain.
