1221826 - update "donate" urls in footer and sitewide header

Hi-

Andrea Wood is going to provide new URLs for the "donate" links in the "sitewide" header and the footer.

Could we please turn that around pretty quickly once she provides the links?

The links are below. PLEASE NOTE: We have to switch the DNS to donate.mozilla.org, which will happen in the next 24 - 48 hrs (bug is here: https://bugzilla.mozilla.org/show_bug.cgi?id=1221736). Until that happens, the form will simply redirect to our existing form (at sendto.mozilla.org). I'll create a dependency as well.

:espressive I realized I made a mistake in the Footer url when I generated it for Andrea, there's no ampersand between the amount and ref parameters. The correct URL should be:

https://donate.mozilla.org/en-US/?presets=100,50,25,15&amount=50&ref=EOYFR2015&utm_campaign=EOYFR2015&utm_source=mozilla.org&utm_medium=referral&utm_content=footer

Also, these URLs don't work yet, waiting on that blocker bug.

Currently the utm_content parameter value will be as follows:

footer => mozillaorg_footer
header => mozillaorg_topNAV

Is that fine or, do you want me to change it to just:

footer => footer
header => header

I used :adamlofting's URL generator which generated the shorter utm_content parameter and he r+'d it: https://github.com/mozilla/donate.mozilla.org/issues/545#issuecomment-154013015

Lets use the shorter parameter.
