I usually watching and working on Army of Awesome for Japanese users. I often see Japanese users who complain about "refresh" discarding installed add-ons. I know, a dialog confirms that, but we also know, most users won't read such warning message. Therefore, I'm not sure if improving "warnings" saves such users. However, at least for Japanese people, I feel "リフレッシュ" ("Refresh" written in Katakana, this word is used in Firefox for Japanese) isn't a good word. That sounds like that only automatically generated part in the profile will be regenerated (i.e., it doesn't sound like no user data (including add-ons) will be modified). So, it might be better to use other words instead of improving warning. E.g., "Uninstall add-ons and reset user settings". # And a lot of users don't realize that backup folder is created in the desktop folder... (bug 921462) I think that this is another cause of this complaint.
I agree that it doesn't communicate the gravity of the action. I was also surprised to find all my add-ons gone. "Refresh" sounds like refreshing a page, i.e. you maintain your current state. At very least "Reset" would be a bit more accurate: gives the impression that you're returning to the initial pristine state. Masayuki's "Uninstall add-ons and reset user settings" would be better still.
FWIW, it used to be "Reset" in english, and was deliberately changed to "Refresh" in bug 1063702, with the wording decision made in bug 1027884.
I don't think we want to scare people away from refresh more than we do now. I think the correct solution is fixing bug 1017919. As a result, closing this in favour of bug 1017919.
