Steps to reproduce:
Open History menu
Middle mouse click an entry
Actual results:
Entry is loaded in current tab
Expected results:
Entry is loaded in a newly created tab
WFM with FF42 on Win 7, middle click opens the entry in a new tab.
Could you test:
1) in safe mode:

2) with a clean profile:

I have now tested to run FF42 in a newly created clean profile. Same result.
As you can see I'm running on Mac OS X (latest public version: 10.11.2). So likely platform specific then.
