I finally trained myself how to do this: click "squashed diff", then click the publish button. With a recent change, I now need to click "squashed diff", then "view reviews", then the publish button (thanks to Steven for showing me how to do this).
The best solution is to be able to publish from any commit.

mozreview: restore review summary link (bug 1231663) r?smacleod We want to make it possible to publish from any commit in a series. For now restoring the review summary link should reduce confusion for people who need to publish a commit or add a reviewer manually.
