I frequently review both an interdiff and the main patch. There should be a way to publish the reviews for all of these in one action as with Rietveld rather than having some of them stick around.
On second thought, maybe this is how it already behaves but the UI is so confusing I can barely tell
Yeah it's a bit confusing, but it works. If you leave a comment on an interdiff between revisions X and Y, it will show up along with any other comments on diff Y when you publish the reviews. You can see everything that will be published when you click "Finish review".
