Steps to reproduce:

Open http://capitalonecareers.com/sites/capitalonecareers.com/files/BenefitsSumMiniguide_new.pdf


Actual results:

Duplicate images and text offset to lower right.

Reproducible with 2016-01-18 Nightly and 45.0a2 and 38.5


Expected results:

Images and text only rendered once, as seen in Chrome and OSX Preview

Only page 1 of the PDF doc has the issue
OSX Preview also has the issue. Adobe is fine. Looks like a layers issue.
