Steps to reproduce:
1. From Mac
2. Block any npapi plugin.
3. Go to page that loads that npapi plugin
4. Click to Allow the plugin
Actual results:
The page did not refresh like on Windows (and other browsers)
Expected results:
The page should have refresh just like it does on Windows

Could you provide more specific steps to reproduce, including a particular URL or testcase? Questions I had from your initial report: does "Block any npapi plugin" mean mark it click-to-activate in the addon manager?
I don't think we refresh the page on any OS, we simply activate the plugin in-place.

You are correct.
Its Safari that refreshes the page when plugin is trusted
