We would like to add a simple sign-up form to the View Source website. The backend will feed into Basket and Brent Walter is assigned to that. This is only for the front-end work of adding the field and accompanying copy. 

I've attached a photo of last year's sign up. I also attached the PSDs for last year's website. You can use the same text and iconography for now. I also have the font files and such if you need them.
