"It's all Text" extension (update June 2015 and working fine until 45 beta) won't believe it's been disabled - see screenshot after 3 restarts (both "restart" link and "Quit")
45 beta has been very sluggish at times for me -- finally today I got a "unresponsive script" from "It's all text" on a github page. (I mostly thought it was gDocs being the problem before, still could be.)

If you look in the browser console after startup there might be some add-ons related messages to explain this. Normally it is a file permissions problem.

Must have been a transient issue -- it's working okay in FF 46 beta. I'll remember to check the console next time.
