In bug 1275040 I had posted a patch which got r+ and landed. Later, someone reported a problem and then I created a follow-up patch and posted it for a new review on the same bug.

This patch was:
 - a new patch on my patch queue (including different MozReview-Commit-ID)
 - based on a different tip (this was a few days later and I had already updated my tree)

However, MozReview thought this was the same patch and:
 - reused the same attachment, changing its name
 - carried forward r+
 - displayed a messy interdiff

All I want was it to treat it as an entirely new patch on the same bug..
