Steps to reproduce:

* Install Mozilla Thunderbird 45.1.1 on Windows 10 (but the issue is not specific to those versions)
* Configure a master password
* Configure multiple calendars (google agendas in my case)
* Restart Thunderbird

Actual results:

Issue #1 - When starting Thunderbird, the master password is asked 12 times. (This is approximately the number of calendars I have configured).

Issue #2 - One of the password prompt windows looks "fatal". Whatever if I type 12 times my master passwords or if I cancel the 12 windows : when closing one of them (or typing the password) => ALL THUNDERBIRD WINDOWS HIDE INSTANTLY (even the main Window with the emails). Then the "thunderbird.exe" process keeps running in the background and I can't do anything as I have no more accessible GUI !

Issue #3 - At this point, I am forced to kill "thunderbird.exe" as otherwise I can't run it any more neither do anything without a GUI.
This done, when I restart TB and try to go to the options (to remove the master password which seems to cause some troubles), I have a BLANK "Options" WINDOW and I can't change any options. => See the enclosed screenshot.


Expected results:

1 - My master passwords should be asked only once when starting Thunderbird
2 - If I type my master password, then all the accounts and agendas should synchronize and the program should be usable normally.
3 - The "Options" window should not be blank, I should be able to change the options.
