Details
There's no way in MozReview to grant an r+ if you are not the original person requested to do the review. This makes it impossible to do drive-by reviews. It also makes it impossible to mark your own patches as r+, either
due to review-on-IRC, because MozReview didn't correctly over r+, and any number of reasons.

Not sure what you mean... Anyone can leave a review on any request. Are you seeing a particular place where you can't?

It just added a second r+, but left the original review request outstanding. The idea was to replace that one.

Ah, so your issue is more about modifying the reviewer list. It's intentional that we don't automatically remove r?s; as with Bugzilla, leaving an r+ and cancelling another person's r? are separate actions. Modifying the reviewer list is being worked on right now, so duping it over there.
