This crash first showed up in Nightly 20160502030207, though it has become a bit more frequent since June 12. It has occurred 24 times overall:

In every case we're trying to execute code at address 0x0. nvumdshim.dll is apparently an "NVIDIA Compatible D3D Shim Driver". The crashes mostly occur very shortly after start-up, but not always.
I don't know how video team decides which drivers to block for D3D11 DXVA.

For D3D11 we just block anything that crashes and fall back to D3D9.

Not too obvious in this long line, but the only diff is right at the end, "; nvumdshim.dll: 10.18.13.6822".
