Due to someone *ahem* ignoring my review requests all week, I've collected a
large set of bookmarks changes, including:

- renaming create[Folder|Bookmark]Ex functions to create[*]WithDetails, and
updating callers.
- persistence of sort data in bookmarks list views across all bookmarks list
views (so sorting one view sorts all)
- persistence of column width and visibility data across sessions
- remove existing dynamic IE Favorites Folder if we do a one-off import of IE
Favorites at start up (to avoid having two Imported IE Favorites folders).
- Removes underlined appearance of links in sidebar.
- Applies uniform single click folder opening behaviour to History panel.

Patch to be attached.

I am seeing a reproducible crash in
nsXULOutlinerBuilder::GetTemplateActionCellFor when switching to bookmarks tab
in sidebar (under certain conditions) - see bug 131247. Could that be caused by
this check-in?
