Steps to reproduce:
Before Firefox 48, I had disabled Unified Complete via about:config option (browser.urlbar.unifiedcomplete). That setting is not respected by Firefox 48.
The topmost "Visit" action of the search/URL/whatever-bar is *almost always wrong* (no hostname, so stored passwords don't work and other amenities like bookmarks or history navigation are just further from the user's reach).
Actual results:
The browser.urlbar.unifiedcomplete is not respected by Firefox 48. Memorised navigation fails and leads to the wrong resources or even wrong services.
Expected results:
Unified Complete functionality should be a readily visible option, or "visit" should be fixed to respect user preferences and habits.

I have tested your issue on latest Firefox release (v48.0) and latest Nightly build (Build ID: 20160811030201) and managed to reproduce it. There are two issues in progress regarding this matter. I have linked them to this bug.

This is by design and thus a wontfix. We have various bugs filed to address users feedback among which bug 1235397 is what would solve this specific complain.
There is also a userChrome fix around that can hide that entry, please ask in users support communities.
