[Flash study] Create dashboard to report on Flash Study

User Story

Each week, a dashboard or report will show the following metrics:
- The % of active users who clicked “allow and remember” at least once during the week.
- The % of active users who clicked “allow now” at least once during the week.
- The % of active users who clicked “do not allow” at least once during the week. (We want this number to be low because we only want to show the prompt for wanted content.)

- A list of domains where either “allow” button was clicked most often.
  * broken down by geo
  * broken down by user feedback option
  * broken down by whether enabling solved the problem

- A list of frequent domains which were CTA but where users did not choose to activate.
  * broken down by geo
  * With the ability to drill down into whether Flash was 1st-party or 3rd-party, and if 3rd-party the domain(s) of the 3rd-party frame and the URL of the SWF itself

- Each week, show a chart of the ratio of (top-level pageloads that attempted to load Flash) / (top-level pageloads).

We need to create a dashboard to do the reporting on the study, according to what's defined in the requirements doc.

This is probably not an appropriate bug for community contribution, just given the timeline and nature of the project.

But probably more importantly, it's already being worked on (in fact it's almost done), we just forgot to note the assignment at the time.
