After latest diagnostics in Bug 1355450 this machine was repaired but seems that the problem occurs again.
Last update was : "diagnostics repaired the disk. ive also reimaged the host. if it fails again, we'll look into RMA."
Please check again this machine and re-image after.

this machine was found online. ill run disk diags again.

no issues found during disk diags, running memtest.

memtest did 3 passes with no issues found. i went ahead and reimaged the host again.
