Aaron, any ideas why SendPDocAccessibleConstructor may fail on document's initial update at [1]?

This is definitely some kind of IPC error... looks like we don't do a good job of annotating that error in crash reports when it happens in content. :-(

there's a dual bug 1383501 (crash PBrowserParent::RecvPDocAccessibleConstructor), I'm curious if it's part of the same puzzle.

Startup crash on Windows, mostly happens with oop clients. Low volume.
