I get it on a regular basis on OS X with the default theme for now, but sadly can't provide good STRs yet, this seems pretty random for now.
As stated in the summary, by using the browser sometimes my mouse pointer go over some tabs and this trigger a favicon redraw.

From now on I'll try to pay attention to every details that might help better understand when is this happening.
To confirm, but I noticed this was happening on unloaded tabs for now.

This also happens sometimes when switching with the keyboard to another tab (ctrl[-shift]-tab).
