In the attached screenshot, the already-known site icon for this site is not used for the first autocomplete entry directing users to this site.

likely we don't have an icon for that, because you didn't directly visit that, you probably visited a document part of that domain, but not the root.
I suggested some time ago to always store the root favicon along with the others, but it would be a slightly high cost for a small benefit (in the past we had other UIs showing domains, like about:permissions that in the meanwhile have been removed).
