User Agent: Mozilla/5.0 (Windows NT 10.0; Win64; x64; rv:58.0) Gecko/20100101 Firefox/58.0 Build ID: 20171101104430 Steps to reproduce: Install an extension that uses a pageAction. Actual results: The pageAction appears on every site. When removed from the address bar, the pageAction reappears when Firefox is relaunched. This occurs with different extensions. Expected results: The pageAction should only appear when the extension specifies it should. The user should be able to remove it from the address bar.

I can confirm that Firefox does not remember when an extension’s Page Action was removed from Address Bar. Steps to reproduce: 1. Install e.g. https://addons.mozilla.org/en-US/firefox/addon/njs/ 2. It will appear as a Page Action at the right of the "Page actions" three-dots button. 3. Right-click and select "Remove from Address Bar" 4. Quit Firefox Nightly. 5. Restart Firefox Nightly. Expected results: The extension’s page action is hidden, and can be accessed in the "Page actions" dropdown. Actual results: The extension’s page action is visible at the right of the "Page actions" button.

There are a couple of bugs in comment 0: bug 1413692 and bug 1413574. I'll dupe this to the latter.
