User Agent: Mozilla/5.0 (Macintosh; Intel Mac OS X 10.12; rv:59.0) Gecko/20100101 Firefox/59.0
Build ID: 20180201171410
Steps to reproduce:
Close a Firefox window then activate macOS Mission Control's "Application Windows" feature.
Actual results:
The closed Firefox window is still shown in the Application Windows, albeit just a black screen. Eventually the black screen disappears, but the window title is still present [see screengrab].
Expected results:
The closed window should not be shown at all in Mission Control.

I'm unable to reproduce this. Does this still reproduce after a system restart? Could you run mozregression[1] to see if this is a regression? If you have never run mozregression before, simply run these three commands in a Terminal window:
sudo easy_install pip
sudo pip2 install -U mozregression
mozregression
A number of Firefox versions will open in succession to narrow down when this started occurring. Simply type "good" or "bad" in Terminal based on whether or not a build reproduces the bug.

I've now replicated this bug in Mail.app so clearly the issue lies with the OS and not Firefox. My apologies for wasting your time.
