We didn't do anything yet to the in-repo docs in fluent.migrate.

bug 1454621, move tools/migrate/README toplevel, fix up README and CHANGELOG, r?stas
README: make PYTHONPATH a bit less cumbersome, and remove the examples, which we
already removed in the repos.
CHANGELOG: Strip changes that didn't affect the migration code.
