When opening Firefox by using a link in an external application, I ended up with an additional blank window below the actual Firefox window. I suspect that this has something to do with session restore and opening Firefox from an external app, but I wasn't able to reproduce it after the first time it happened.

Given that you can't reproduce and that your screenshot shows the green arrow icon indicating an update will happen at the next restart, I'm very tempted to assume that you had an outdated nightly and were experiencing bug 1448135. So unless you find a way to reproduce with a current Nightly, I'm tempted to mark this as a duplicate of bug 1448135.
