1462569 - Add an `entrypoint` to query parameters when redirecting users to FxA

The new ActivityStream overlay that asks for the user's email and redirects users to FxA does not include any parameters that can be used to attribute users to a source. At a minimum, an `entrypoint` should be sent, perhaps `entrypoint: activity-stream`.

utm_* parameters can also be sent for more fine grained info, e.g., utm_source could be `activity-stream` and utm_campaign `startup-overlay`.

I was just re-reading the purpose of https://github.com/mozilla/activity-stream/pull/4090 and thought of more descriptive values:

entrypoint: `activity-stream-firstrun`
utm_source: `activity-stream`
utm_campaign: `firstrun`
