1506725 - Update form.doc to direct reporters to GitHub

We need to remove the HTML form and replace it with a notice: "The MDN Content team has moved the process for managing documentation requests to GitHub. Please file new bugs on [GitHub](https://github.com/mdn/sprints/issues/new?template=issue-template.md)."

Janet, please let us know any changes to this text and an appropriate page heading.

Alternatively we could silently re-direct requests for the old form to GitHub.

This text is good. 

If it's not too long, I think the page heading should be "Dev Doc requests have moved". 

Would rather have the page (not a silent redirect), even if it requires an extra click for users, to be transparent about what's happened, and give them the explicit link to use in the future.

Please let me know when this change will be pushed live. I want to update links on MDN roughly concurrently, so we minimize the time when we are getting issues from both bmo and GitHub.

Yes, please. "Developer Documentation" refers to MDN content and is the module that should change. "developer.mozilla.org" is the website itself, and is NOT moving off Bugzilla.
