When filling the Beta uplift request for https://bugzilla.mozilla.org/show_bug.cgi?id=1515205 me and dminor both filled out the new form with the questions and answers. But after submitting the request the bug only showed the beta uplift flag without any comment.
Ryan pointed out that https://bugzilla.mozilla.org/show_bug.cgi?id=1507103 probably shows the same problem.

This turns out to be a regression from Bug 1520533. I only tested locally with a regular patch, but Phab requests are something different, hence the bug. Sorry about that!
