Steps to reproduce:

go to www.confirmit.com. Click on Contact on right hand side of screen. The window opens and it is blank.

Actual results:

The form that should be on the side screen is not there the window is blank.

Expected results:

There is a form to fill out on the screen if you go to the site in Chrome.

You have an adblocker extension installed and that is the reason why the contact area is empty.
I do not see any problems with Firefox65 on that page without adblocker.
