I've seen this today when I tried to enter 9gag.com and it crashed (probably due to bug 1532054). See attached.
Not sure how to trigger it on older builds, so I can't check what the regression range is.
Is there an easy way to force a tab to crash?

I believe you can use your OS's process manager to kill one of the content processes.
I'm unable to reproduce now that bug 1532054 was fixed (even after killing content processes -- the crased tab icon is in the right place), but I'm sure the underlying issue is still there.
Prior to bug 1532054 I could reproduce the issue but intermediately.
