I have an Herald rule that subscribes me to any diff including l10n changes.

This diff is set bmo-mozilla-employee-confidential, for which I have access to, but Herald never subscribed me to it

The Herald rule did actually add you to the revision at:

https://phabricator.services.mozilla.com/D20149#602376

But then the BMO code we use to sync revision data with private revisions and private bugs removed you right after.
You need to be in the CC list of the bug if you want to be added to the subscriber list of a private revision. Permissions
and subscriber list is manage by BMO for private stuff. This is by design and not likely to change for the near future
unless a better workflow is discovered.
