In bug 1528690 the number of crashes increased sharply a few months ago. In bug 1546892, I suspected one of the main reasons behind is that previous StartTransactionOp failed so that the next ObjectStoreAddOrPutRequestOp didn't have an update refcount function.

We should figure out where it failed exactly in this bug. Probably, telemetry to track the number of failure in functions would be a good way to do.
