It was supposed to follow a survey you made on Firefox users.
I don't know whether you actually initiated that survey, and i don't know where to report what i suspect is a phishing.

hi, this is certainly not something that's run by firefox, so please do not provide any of your personal details or payment info there.
you can report malicious sites that are impostoring firefox or mozilla at https://www.mozilla.org/about/legal/fraud-report/ and phishing attempts through https://safebrowsing.google.com/safebrowsing/report_phish/?tpl=mozilla
