Blocklist bug form is missing a bug type field

The bug_type field now seems to be required and does not default to the component's default if not specified. The blocklist creation form does not include it. This could either be fixed by using the default for the component if not specified, or adding the bug type field to the form.

When reviewed I'd appreciate if we could push this outside of the normal cycle, as it is blocking people from filing new blocklisting bugs.
