Incorrect/phantom bookmark and autocomplete suggestions?

Steps to reproduce:
- Start to type site into address bar
- View autocomplete suggestions
Actual results:
Suggesting incorrect sites that appear to be bookmarked (with a ★ star icon) but definitely aren't, and definitely aren't part of browsing history.
Can't delete related autocomplete entries with highlight and Delete or (Shift+Delete).
Expected results:
Suggesting sites that are closer to the typed query - example typing 'reddit' should suggest reddit.com and not redditblog.com, or typing 'youtube' should suggest youtube.com and not youtubedownloader.com.

SOLVED - after further digging, it seems like old/NULL URLs had for some reason been stored in places.sqlite. After checking records from both the old and new places.sqlite files, I've gone from 372 NULL/phantom records to only 11:

It's possible that backing up and restoring bookmarks would achieve the same result, but I only tried the above first:
