I'm going to send a couple attachments showing the Add Bookmark dialog the 1st
time I use it in a session, and after resizing it.

This started happening after a general system crash, and seems to have gotten
worse after each crash. (Bad hardware. I'm saving up for a new rig.) Crashes
also preceded beginning of behavior in Bug 152442.

Attachment: Self-corrected after 1st resize of dialog.

This is how it looks after I resize it.  Subsequent uses appear normal.
