Because our timeline is tight, and there are Voice project dependencies and OKRs on this survey going live. Our ideal launch date is just over a week away.

HI Emma,
If someone answers "no" to the age gate question, they should be popped out to a thanks but you can't participate page OR the age disclaimer should be at the bottom of the page with a clear statement that someone has to be 19 or older to participate. I thought this part of the surveygizmo template, but maybe not?
Also, the optional email address field should be last after the other questions and preferably on its own page. We prefer to have optional fields like that at the end so that people can participate in the questions and not feel like they have to provide their information.
Also adding MFeldman to review from the legal side for language/wording etc.

language/wording looks fine to me. but +1 Alicia's comment above.
I am not sure about a SurveyGizmo template? I built this from scratch anyway...
I have added logic, which if someone answers 'no' to being over 19 redirects them to a disqualification page with the text "Sorry, you do not qualify to take this survey. Participants must be 19 years of age, or older."
I also moved the email address field to the end, and placed it on its own page (Titled 'Thank You!' ).
I am also missing a blurb about privacy policy, is there a standard one I should use?
I figured out how to add the template, that has the privacy policy and age requirement.

Hi Emma,
Tyler Downer should be able to help with the standard template so that you don't have to do that manually and it includes the age disclosure and a link to the right privacy policy.
You can also contact the survey team for help in Slack at #surveys if Tyler is unavailable.
When that's all done, I'm happy to do a final check for you.

NM - I just saw comment 7. I've double checked the survey and it looks fine from the data privacy side.
One note: you have a question about using existing solutions with an option for a write-in box, but there is no text field to go with it. Looks like you might want to add a field.
