This was due to a targeting issue that required the user to not be logged in. The new updated targeting is "firefoxVersion >= 71 && userPrefs.cfrFeatures" and has been deployed to Remote Settings production.

I have verified that this issue is no longer reproducible with the latest Firefox Beta (71.0b9 Build ID - 20191111170815) and the latest Firefox Nightly (72.0a1 Build ID - 20191114055112) installed, on Windows 10 x64, Arch Linux and Mac 10.15.1. Now, the "PDF_URL_FFX_SEND" recommendation is successfully triggered when accessing a URL that ends in ".pdf".
