If we can track the completion page, we should not prompt again. otherwise we should probably show it a max of 3x or something.
I talked with jgaunt about how the add-on can detect when the user completes the survey. One way is if the survey loads a new page/URL on completion. The add-on could keep a tabs listener and listen for the URL. jgaunt is looking into whether that's possible. Another way is for the add-on to use a content script on the survey page and listen for a thank-you message or button click or something.

We should sniff not for reaching the completion page, but instead for leaving the first page. (If someone answers the first page of the survey and then quits before completing, we don't want to get data from them a second time)
