I'll try to get better steps to reproduce but I wanted to get this down so I
didn't forget.
I can get in a state where dragging an item off of the toolbar and back onto the
icon selection area results in the "restore default set" button moving up into
the icon selection area.
To achieve this state just add and remove items from the toolbar. Throw in a few
reorders for good measure. It might be necessary to drop icons in different
places on the palette as you drag them out of the toolbar.
Once in this state it is pretty easy to get in a worse state where the toolbar
will become empty collapse vertically and the icon selection area will either
become empty or the icons will be overlapping and stacked in a vertical column.

To reproduce this, just drag from the toolbar and drop right at the top of the
palette.

steps to repro:
1.with the default toolbar configuration open the Customize Toolbar window.
2.drag the back button down slowly and drop it just at the point that you first
get a drop cursor (below the "drag and item...." text and above the separator
button)

verified with a build from this evening on win2K and RH null. Dropping at the
top of the palette no longer horks things. I've also tested by just dragging and
dropping icons randomly for about 2 minutes. No horkage.
