OFN_EXTENSIONDIFFERENT
Specifies that the user typed a file name extension that differs from the
extension specified by lpstrDefExt. The function does not use this flag if
lpstrDefExt is NULL.

I suspect that our extension fixup code is ignoring this flag.

However i haven't spent the time to prove it.
