There appears to a serious bug in the enumeration of filter categories. The bug effectively discourages the addition of custom filter categories. Adding a custom filter category causes the replication of that category plus the addition of another Undefined item every time Mozilla application is started. The progression appears geometric, a doubling of the instances of all custom filers and Undefined items. So 2, 4, 8, 16, 32, 64,... This effectively disables the use of custom filters as the list quickly grows too long to be usable. The user's only choice is to remove all custom filter entries.
Did you use a colon ":" when adding the custom header? (bug 107620) What build are you using?
Laurel, I just read description on #107620. This is likely a dupe bug. The behavior and likely cause described in #107620 maps exactly to my experience. Build, Mozilla/5.0 (Windows; U; Windows NT 5.0; en-US; rv:1.1) Gecko/20020826
