In testcase #1 and #2, the <div> is "top-of-page" so its forced break should not be honored. The underlying issue is that the design of forced page breaks in Gecko is broken, so this basically amounts to fixing bug 683043. However, it's likely there are also a few things that needs to be fixed in nsLegendFrame/nsFieldSetFrame::Reflow.
