Require a comment when changing the severity field.
- requires client-side notification
- transient top of page notification
- "A comment is required when changing the severity field. Please use this as a guide."
- pre-populate comment field if currently empty
- "Changing severity to S? because of <rationale>."
- prevent submission of bug where the comment is identical to the template
- transient top of page notification
- server-side enforcement with error message if comment is not provided
- "A comment is required when changing the severity field."
